The Geriatric, Rehabilitation Medicine and Extended Care HCG (GRMEC) bring together a diverse group of professionals and non-professionals who are dedicated to the care of the older and physically challenged veteran. Their expertise is called upon to address the acute, chronic, preventive, rehabilitation and maintenance care needs for individuals who often have multiple medical diagnoses.

The settings for care include the Nursing Home Care Unit (NHCU) and the Geriatric Evaluation Management (GEM) Program that includes inpatient and outpatient services.

Rehabilitation services are provided in both inpatient and outpatient locations. They include

• Physical Therapy
• Occupational Therapy
• Audiology and Speech Pathology
• Kinesiotherapy
• Vocational Rehabilitation Therapy including Compensated Work and Incentive Therapy
• Preservation Amputation Care and Treatment (PACT) and Recreation Therapy

Specialty services include:

• 15-bed inpatient Hospice unit
• Respite Care program
• 4-bed inpatient GEM program
• 10-bed inpatient acute Rehabilitation unit

which are all located in the NHCU.

Other settings include the services provided through Community Based Services, which is comprised of the Home Based Primary Care (HBPC) Program, Contract Nursing Home Program, Geriatric Residential Care, Care Coordination/Home Telehealth and private home care.
